In the November issue of Electronics Products I stumbled upon the LT6016, LTC2057 and LTC6090 "over-the-top" and high voltage op amps.
The LTC6090 can operate on a 140 volt rail.
LT6016 has an input common mode range that is 76V over it's negative pin.
The LT2057 operates up to 60V.
As you might expect the bandwidth of these parts is too low for audio but they might be suitable in other applications such as HV servos.
The input-capacitorless preamp common mode servo is one example.

Page three of the selection guide shows Linear's selection of op amps that operate on >30V supplies.
Many of those parts are suitable for audio e.g the LTC1028 etc.
